A year after lavish funeral, Savile myth in ruins

LONDON (AP) ? At Jimmy Savile's funeral, a year ago Friday, the priest delivering the homily was emphatic: the DJ and television host "can face eternal life with confidence."

Hundreds of people packed a cathedral for Savile's funeral Mass, thousands paid their respects at his coffin, and people from Prince Charles to the Bee Gees sent condolences. He was a cultural fixture, even an icon, and his BBC television shows had been part of childhood for two generations of Britons.

But a year on, Savile's reputation is in ruins. Police have branded him one of Britain's worst sex offenders, accused of assaulting underage girls over half a century. Like those who feted and praised him on that November day, millions are wondering: How could he have duped so many for so long?

A SELF-MADE MYTH

"His life story was an epic of giving ? giving of time, giving of talent, giving of treasure," Monsignor Kieran Heskin told hundreds of mourners at the funeral. "Sir Jimmy Savile can face eternal life with confidence."

Savile's death, like his life, was full of self-spun mythology. He cast himself as a colorful entertainer who worked tirelessly for charity ? and he choreographed his exit as carefully as an Egyptian pharaoh, leaving instructions for an elaborate three-day commemoration in his home city of Leeds, in northern England.

Thousands of people turned out to pay tribute at the Queen's Hotel, where the entertainer's coffin sat surrounded by flowers, photos and the last cigar he ever smoked. Inside lay Savile, dressed in a tracksuit and clutching a string of rosary beads.

Others lined the street as Savile was carried into St. Anne's Cathedral by Royal Marine pallbearers for a richly ceremonial requiem Mass. Later he was buried in a golden coffin, in a tree-shaded cemetery ? and on a 45-degree angle so he could overlook the sea.

CHARITY SHIELD

"He had gold, jewelry and diamonds, but wealth meant nothing to him," Alistair Hall, a cardiologist at one of the hospitals Savile supported, said in his eulogy. Savile, he said, "was as he appeared ? a caring man."

Savile cultivated the persona of an eccentric, curmudgeonly but generous uncle. He wore brightly colored tracksuits and chunky gold chains and drove a Rolls-Royce. On the long-running TV show "Jim'll Fix It" he made children's wishes come true. Off-screen, he ran marathons for charity and frequently visited schools and hospitals.

What now seems clear ? what so many missed ? is that both roles brought him into contact with potential victims: star-struck teenagers, vulnerable patients, inmates of a secure psychiatric hospital.

Cary Cooper, a professor of psychology at England's Lancaster University, said that probably nobody will ever know whether Savile used his charity work deliberately to meet victims, or simply to burnish his saintly image. "Either way," Cooper said, "it protected him more, being seen as a philanthropic individual. It served his purpose."

At the funeral, Hall said Saville's charitable legacy would live on. Last month the trustees of two charities that bear his name announced that they were closing down.

FRIENDS IN HIGH PLACES

When Savile died, Prince Charles' office said the heir to the throne and his wife "were saddened to hear of Jimmy Savile's death."

The late DJ boasted of his ties to powerful people, including Prince Charles, the late Princess Diana and former Prime Minister Margaret Thatcher, whom he visited at her country retreat.

His connections may have helped shield him from criticism. Several young people accused Savile of abuse while he was alive, and he was questioned by police, but no charges were laid ? and no newspaper ever printed the allegations.

Now, police are investigating claims of abuse from some 300 people who have come forward since the scandal exploded when allegations about Savile were broadcast in a TV documentary in early October. And police are facing investigation themselves for their failure to act sooner.

Charles' Clarence House office says the prince's relationship with Savile was solely a result of their shared charity work.

REMORSE AND REGRET

"If there's a heaven, he'll be laughing now if he's got time," fellow DJ Tony Prince said at the funeral. "Because if there is a heaven, he'll be introducing Elvis on the clouds."

Younger DJs mentored by Savile were out in force at the memorial, and remembered the flamboyant star fondly. One, Dave Eager, wore a bright yellow sweat shirt saying "Jimmy's Eager Helper."

"Everyone who knew Jimmy knows it was a life-changing experience," he said.

Last month, Eager told The Sun newspaper that he was "completely and utterly gobsmacked" by the allegations against Savile, and felt guilty about failing to stop the abuse.

"You feel traumatized and sorry for the people abused by Jimmy but equally you think, 'Why the bloody hell didn't we see something?'" he said.

A SHATTERED IMAGE

Savile's carefully crafted myth didn't outlive him by long, and he has not rested in peace. His family has had the star's gravestone destroyed in response to public outrage. This week his nephew backed calls to exhume and cremate Savile's body out of respect to other bereaved families.

Of all the words spoken at the funeral a year ago, one comment now sounds prophetic. "None of us really knew the real Jimmy," fellow DJ Mike Read said. "Maybe he didn't even know himself."

Draghi open to ECB rate cut, done helping Greece

FRANKFURT (Reuters) - The euro zone economy shows little sign of recovering before the year-end despite easing financial market conditions, European Central Bank President Mario Draghi said on Thursday, leaving open the possibility of an interest rate cut in the months ahead.

But after keeping rates on hold on Thursday, Draghi said the ECB cannot do much more to help Greece with its debt burden and gave Spain none of the assurance it wants that ECB bond buying will lower its borrowing costs.

"The ECB is by and large done," Draghi told his monthly news conference when asked what the bank could do for Greece.

The euro zone is grappling to find a formula to make Greek debt sustainable, with Germany and the International Monetary Fund at odds over the need for governments and the ECB to take a "haircut" on Greek bonds they hold to make the numbers add up.

The ECB agreed earlier this year to hand over to euro zone governments profits on its Greek bonds but has refused to take a hit on the value of the paper, saying that would be "monetary financing" which it is prohibited from doing.

The ECB held its main rate at 0.75 percent, deferring any cut while it waits for a cue to use its new bond-purchase plan. That wait may be prolonged after Spain completed its 2012 funding at affordable rates on capital markets on Thursday.

A Reuters poll had given an 80 percent chance the ECB would hold its main rate, but most of the 73 analysts polled expect it will be cut to a new record low of 0.5 percent within the next few months.

Draghi said ECB monetary policy is "very accommodative". He declined to comment when asked whether markets were right to expect a rate cut next month and said the policymaking Governing Council had not discussed what it would do next year.

Economist Howard Archer at IHS Global Insight said: "Draghi appeared to ease open the door to a cut in interest rates over the coming months and potentially as soon as December."

Not everyone expects a cut that soon.

"Our sense is that the ECB is firmly on hold," said JP Morgan economist Greg Fuzesi, though he added: "Next year, the ECB will act if growth disappoints more fundamentally."

Describing "a picture of weaker economies" in the euro zone, Draghi said this would influence new ECB economic forecasts due next month. Inflation would remain above the ECB's target for the rest of the year, before falling below 2 percent in 2013.

"We certainly continue monitoring economic activity and we stand ready to act," he said.

"We stand ready to act with OMT (bond-purchase plan) once the prerequisites are in place. We also stand ready to act with the rest of standard, normal monetary policy instruments."

OMT READY TO GO

Recent survey evidence gave no sign of improvement towards the year-end and the risks surrounding the euro area remain on the downside, Draghi said. As he spoke, the euro fell against the dollar and hit a session low in early New York trade.

Gloomy data this week indicated the euro zone economy will shrink in the fourth quarter, which the ECB could eventually respond to by cutting rates.

Before making any decision to cut rates further, the ECB will focus on making sure that its looser policy reaches companies and households across the euro zone, a mechanism that has been broken by the bloc's debt crisis.

The new bond-purchase plan - dubbed Outright Monetary Transactions (OMTs) - is the ECB's designated tool for this but can only be activated once a euro zone government requests help from the bloc's rescue fund and accepts policy conditions and strict international supervision.

So far no request has been made, but the announcement of the policy alone has calmed markets.

"We are ready to undertake OMTs which will help to avoid extreme scenarios, thereby clearly reducing concerns about the materialization of destructive forces," Draghi said.

Asked whether he could imagine an extreme scenario in which the bank began buying bonds without conditions, he said the answer was 'no'.

CALLING FOR HELP

Investors and euro zone policymakers have been urging Spain to seek aid but Prime Minister Mariano Rajoy has so far held off a request, saying he wants assurances that ECB intervention would bring down Spain's debt costs.

Draghi gave Rajoy no comfort.

"The Governing Council will take the final decision in total independence," he said of any decision on whether to use the OMT programme. "In so doing, it cannot give any assurance ex ante".

Spain sold 4.8 billion euros of debt including its first longer-term issue in 18 months on Thursday, enough to complete its 2012 financing programme and begin raising funds for next year. So there is little immediate pressure on that front.

Yields on Spanish government bonds have dropped by around 2 percentage points since Draghi said in late July the ECB was ready to do "whatever it takes to preserve the euro" - a pledge that heralded the bond-buying plan.

Investment funds have started flowing back into the euro zone since then, particularly from U.S. money market funds, Draghi said.

Some economists have now raised the possibility that the OMT might never have to be activated considering its impact so far.

But Matteo Cominetta, European economist at UBS, said it would eventually be put to the test because of the large amount of Spanish sovereign debt coming up for refinancing next year, roughly 140 billion euros according to Reuters data.

"Next year, you will have a record supply of Spanish bonds up for renewal in a situation where macro economic data will remain very bad for a long time in Spain," Cominetta said.

This by my colleague and regular co-author from the UBCMJ | SEPTEMBER 2012 4(1). As you will see, Dave has a rather unusual quality of mind in that he has a philosophical sensibility and so many reference points that typically few with his training would have.

Two major phenomena have driven dramatic changes in medicine and healthcare careers over the past century. One critical factor has been the almost logarithmic increase in knowledge, a general phenomenon forcefully evident in medicine over the past decades. The other significant phenomenon relates to social and cultural changes with gradual development of more accepting attitudes in a diverse medical and healthcare workforce. Each of these is paralleled by consequential, organizational, family, and institutional changes.

Knowledge has expanded at about 2% per annum, compound, over the past 200 years, according to economic studies using translation of knowledge into productivity increases as a surrogate measure.1 Creation of new knowledge has also led to innovative translation with huge changes in medicine. Whereas 100 years ago a single brain, that of Sir William Osler, could be the primary repository of what was known then in medicine and write the comprehensive textbook ?Principles and Practice of Medicine?,2 today, one single mind cannot contain all that is known in medicine. Accrediting bodies such as the Royal College of Physicians and Surgeons of Canada and the American Boards of Medical Specialities in the USA now recognize over two dozen specialities, each of which requires a whole career focus and is in itself a brain full of knowledge. This profound increment in medical specialities continues with new subspecialties proposed each year. Similar proliferation of roles have led to an expanded number of new professional careers in nursing, pharmacy, physiotherapy and occupational therapy, and other healthcare activities ? unlike years ago where each was a single profession.

Nowadays, this requires collaboration facilitated by electronic communications with practitioners cooperating as inter?professional ?communities of practice? in the best interests of patients. Additionally the patients and practitioners are now exposed to web based knowledge that accentuates the requirement for caregivers to also have immediate access to detailed current knowledge. It also emphasizes the need for all practitioners to use continuing education programs to maintain an up to date knowledge base! This, together with patient-rights initiatives, is gradually leading to a balance with physicians being seen as having expertise and authority over medical therapy and patients exercising more control over which options for care are implemented according to their preferences.3 This is, however, a very complex set of interdependent professional relationships and will continue to evolve long into the future.

Fifty to one hundred years ago, single, apparently omniscient physicians, nurses, or others with comprehensive knowledge in their specialty or practice often presented themselves as authoritative, occasionally irascible bosses with structural authority.4 Such prescriptive behaviours often occurred in operating rooms or emergency rooms where definitive verbal orders were given with instantaneous action and compliance expected of subordinate nurses and others. Nowadays, there are numerous knowledgeable practitioners, each more accustomed to assuming the role of helpful expert to assist their teams, who display their own extensive knowledge as ?sapiential? or ?personal?4 authority, a much more benign professional presentation.

From a social and cultural perspective over the past century, major cultural shifts in Western-oriented civil societies have occurred: perhaps as a result of two World Wars and post-recession episodic changes. Changes in traditional family roles has been one of these shifts, with more women in the workforce at senior level workforce positions. If one reviews the pictures of UBC Medical graduates over the past 60 years in the Webber MSAC Alumni room, the number of female graduates in the 1950?s was 5-10% each year whereas in current classes 50-60% of graduates are female. Also, the racial makeup of classes has changed to more closely reflect population numbers. To a degree, these shifts seem to reflect a more balanced perspective towards a civil society with a move away from authoritarianism, thus encouraging a more egalitarian professional situation.

All of these social, cultural, and epistemic changes have impacted the overall orientation of healthcare and medical practice. While always required, added emphasis is now placed on empathy, dependability, and integrity as attributes required of our graduates and coworkers in addition to an ongoing need for critical analysis and reasoning skills leading to measurable clinical competence. This is not to imply that a focus on social issues never existed. I can personally recall having a medical school course on social and cultural values in the 1950?s and still remember and utilize the concepts today of a required essay5 where I proposed a balanced civil society that valued individual freedoms. Condemned was the authoritarianism of the ?soul?less?, and absolutist anarchic or regimented states!

A further cultural shift has been towards an increasing emphasis on the need for economic efficiencies. Indeed, in?medical research, the requirement for effective ?bench to bedside? translation is emphasized today with increasing calls for ?ROI? or return on investment. This has led to the need for effective ethics review committees to protect patients? interests6 and reduce pressures on medical researchers from undue market involvement of commercial entities such as pharmaceutical companies.7 A further problem arises with the increasing clash of professional and corporate interests. A professional by definition practices medicine to the standards of their profession in their community?not as an ?occupational? working to the imposed standards of a boss!8 Pressure is exerted by some healthcare administrators for doctors to practice according to administrative budgetary requirements, thus being more efficient and perhaps less effective in care of their patients. A number of years ago at the Canadian Red Cross Blood Services, the physician in charge appeared to have rejected blood testing for donor blood infections due to testing costs on the orders of his funding agency bosses?in that case provincial government bureaucrats. His unprofessional behaviour was ultimately outlined in the Krever Commission Report.9 This led to him being professionally discredited, the Red Cross Blood Transfusion Service being terminated, and the Canadian Blood Services Agency being created.

In medicine, a major initiative that has had a profound impact is prevention of infectious diseases by immunization. This and other preventive measures have led to the survival of many who might otherwise have died. Now we are faced with an aging demographic, many of whom through excess nutrition and salt intake,10 suffer from obesity,11 hypertension, diabetes, and major cardiovascular issues. These will require additional attention in the decades to come!

So, what is the ultimate answer to the question posed at the beginning of this polemic? The enormous increase in medical knowledge, adapted to improve the quality of care of patients has led to an essential, major proliferation in types of healthcare providers. This in turn has led to significant shifts in professional relationships, not only of patients and doctors but also among healthcare providers. It is interesting to observe that such interdisciplinary activity is effected, based on a principle outlined by Adam Smith in the 1770?s as a significant concept of the Scottish Enlightenment?the Division of Labour.12 In that description, workers provided a defined activity based on their personal expertise and skill that built upon the different expertise and skill of other providers towards an ideal cooperative outcome. In Smith?s description, that outcome was the efficient manufacture of pins; in our current description for healthcare providers it is towards the optimum outcome of care for our patients in an increasingly complex healthcare environment. Cooperative inter?professional care will require greater provider empathy for all to understand and enable each other?s role in these complex, beneficent patient care related undertakings.

'16 and Pregnant' teen regrets abortion

When Jamie McKay first became pregnant, the struggling teen took her story to the MTV docu-series "16 and Pregnant." There she shared the details for the benefit of other young women who might find themselves in similar situations. But as it turned out, after having her first child, it was McKay who soon found herself in thatvery same situation again.

At 18, Jamie was pregnant, but this time she made a different decision. She still shared her story with others, albeit in a very different way -- via Twitter, but she didn't go through with the pregnancy.

Last week, she posted an ultrasound image (which she later deleted) and wrote, "Rest in peace little angel. September 17, 2012."

The message caused some confusion for her Twitter followers, who at first seemed to believe she'd suffered a miscarriage. In a series of tweets that followed, she cleared up the confusion.

"Thank you all for the love and support but I want you to know I did not miscarry," she wrote. "I chose abortion. I chose it out of fear. I feel like I made a mistake and I can't take it back. I have had this in my heart and it has been very hard on me. I was afraid to be judged by my family, my mother, even my dad. So I did it before they could find out. I should have looked into my options more but I didn't."

Reactions among those who read her story ranged from supportive to angry, but Jamie hasn't shied away from any of them. She continues to respond to followers and often retweets messages from those who've commented.

According to Jamie, Ryan McElrath, her boyfriend and the father of their daughter, Miah, "has been supportive through it all." And, "in case you all were wondering, he is the father of that child too. And he let me make my decisions."

While Jamie expressed regret about her choice, she later added, "In my heart I know I made the right decision. But sometimes I wish I hadn't."
